POSITION SUMMARY

The Participant Data Outreach Assistant will perform a variety of duties involving entering, confirming, and updating patient service support documentation. This will include frequent communication with client's and collaboration with various teams within to effectively collect and process participant data.

• • Support data entry of provider information into Care Compass EMR database, including information related to participant home care services received, supplies delivered and other updates as necessary.
  • Complete outreach to clients for confirmation of services received and data collection of missing documentation, etc.
  • Work closely with PACE Service Coordination and Care Delivery department to coordinate and meet appropriate deadlines as a team.
  • Provide general administrative support as needed.
  • Maintain departmental quality and productivity goals.
  • Adhere to all applicable compliance requirements and the Code of Conduct.
